Navigating the world of disability services can be confusing. All you know is that you or a loved one needs help. Sometimes you may know what you need, other times you have no idea. You have so many questions. Where do I start? What is available? What do I need to do to qualify for available services? Frontier Community Services (FCS) would like to help you answer these questions and help you find the available services that will allow you or your loved one who experiences a disability to live in the home and community of their choice.

 Medicaid Services
Green Square TEFRA Medicaid
This is not really a service, but is a way for a child to be determined eligible for Medicaid based on the child's income not the family's income. Your child would have to need a very high level of care that normally would be provided in a facility like a nursing home or extended care center.

Medicaid Home and Community Based Waiver Services

These are services available to individuals of all ages that meet certain criteria. First off you must be Medicaid eligible and need services that would normally be found in an “institution” such as nursing home or extended care center. The types of services that are offered are dependent upon the type of waiver you qualify for. The state of Alaska currently has four types of waivers:

  1. Alaskans Living Independently
  2. Adult with Physical and Developmental Disabilities
  3. Children with Complex Medical Conditions
  4. Intellectual and Developmental Disabilities

All four types of waivers provide the following services: Back to Listing
  Green Square Care Coordination Services

Frontier Community ServicesCare Coordinators work with you and your family to create a plan each year that identifies what services you need, why you need them and how often. The care coordinator then meets monthly with you and your family to make sure that the services are meeting your needs and can make changes to your plan if you need something different.

  Green Square Respite Care

Frontier Community ServicesCaring for someone is hard work and we all need a break. Respite care is designed to give you a break while ensuring your loved one is well taken care of. FCS has people who can come into your home to care for the person you love while you go out and shop, go the doctor or just relax.

  Green Square Chore Services

Fr’ontier Community ServicesWho doesn't need help cleaning the house and getting the chores done? Cleaning your house is a chore and having a disability can make it almost impossible to get done, so FCS can send someone in to your home on a scheduled basis and do basic housekeeping for you.

  Green Square In-Home Supports

Frontier Community ServicesThis service happens in your home and focuses on everyday living skills to maximize your loved one's independence. Staff from FCS would come into your home and work with your family member as they move through everyday routines such as dressing, meal preparation, cleaning etc.

Additionally, the Children with Complex Medical Conditions waiver and new term waiver can provide: Back to Listing
  Green Square Nursing

Frontier Community ServicesChildren with a complex medical condition and some folks with a developmental disability and a medical condition need the services of a nurse to maintain their health. A nurse can come to your home and assist with medical procedures and train the family and care givers regarding the health care of the person.

  Green Square Supported Employment

Frontier Community ServicesAt some point in our lives getting a job is normal. For people with a disability this can be challenging. This service focuses on helping the person develop employment skills, find employment and, once employed, helping them maintain that employment.

  Green Square Day Habilitation

Frontier Community ServicesWe all need to get out of the house and take part in things going on in our community. This service is meant to get your loved one out of the house and involved in social activities. Staff would help with social skills, establish what the person likes to do and then work with your loved one to help them be
successful at it.

  Green Square Supported Living

Frontier Community ServicesWhen we reach a certain age, typically we find our own place to live and move out of our family home. When you struggle with a disability, it can be a challenge to be independent. This service provides individuals 18 years and older a way to develop independent living skill. Staff work with the person in their own home and teaches them cooking, grocery shopping and all of the little things it takes to be as independent as possible.

  Green Square Assisted Living Homes

There may come a time when you can no longer safely take care of the person in his home or even in your home for any number of reasons. At the same time a nursing home isn't called for either. This is where one of FCS' adult assisted living homes comes in. Our homes are just that, homes in regular neighborhoods. They are small (five people at the max living in the home) comfortable houses that the people living there make their own. Specially trained staff are in the home 24/7 to care for the people living there.

  Green Square Grant Services

Frontier Community ServicesIf you don't qualify for Medicaid or waiver services you may be able to receive some of the above services from the Community Developmental Disabilities grant. These grant dollars will cover case management, respite, supported living, day habilitation and supported employment.
